My notes for mySQL 3.x.x - guessing that 4 works similarly...

did you initialize the db first?

mysql_install_db

then start mysql (or mysqld_safe) - I generally use Red Hat start
scripts...

then lastly...set primary user...

mysqladmin - u root password 'new-password'
